Stanford Medicine Children’s Health is dedicated to improving health care through six important focus areas:
- Person-Centered Care: We make sure every patient and family feels important, offering care in their preferred language with the help of qualified bilingual staff and interpreters. Our Family Advisory Councils and Parent Mentor Program bring diverse voices into our operations to improve services for everyone.
- Patient Safety: Our organization works to reduce patient harm by using proven safety practices. We’ve already cut serious safety events by 79% and aim for an 83% reduction soon. We focus on helping vulnerable groups by closely tracking and improving their safety.
- Effective Treatment: We focus on giving all patients high-quality care. We use special clinical pathways to ensure that treatment is evidence-based, helping avoid bias. Programs like the Packard Clinical Pathway are continuously improving care for conditions like Kawasaki disease and sickle cell disease.
- Care Coordination: We support patient care transitions from hospital to home, especially for those with complex needs. By refining discharge processes and improving communication, we aim to provide a smooth and supportive experience.
- Access to Care: Our partnerships with schools, hospitals, and community organizations bring quality care closer to families. By working with local networks like Gardner and Ravenswood, as well as regional hospitals like Dominican Hospital and Salinas Valley Health, we ensure that care is both geographically and culturally accessible. We’re also using new tools to tackle missed appointments and improve access for all.
- Patient Social Drivers of Health: We identify and address social factors like food and housing needs that impact health. We screen and connect families to community resources through the Find Help platform, ensuring that everyone gets the support they need.
